Also, we offer Case Management for all aspects of your care. This Includes Coordination of Physician Orders, Appointments, Medications, and Medical equipment. Our Diabetes Education Care
Management Program in the Home:
- Teaches you how to monitor your blood sugar levels and recognize signs of
diabetes-related complications - Provides diabetes teaching on healthy habits, including lifestyle changes,
wellness, diet, exercise, and medication management - Teach you healthy habits to control your glucose levels and prevent
complications - Empower you to understand and monitor diabetes symptoms
- Help you manage your medications
- Improve functional strength and mobility
- Establish a daily exercise routine that matches your pace and ability level
- Reduce hospitalizations related to diabetes complications
Cardiopulmonary:
Our specially trained Nurses work closely with your physician to manage Heart and Lung health. This can include daily in-home monitoring and Medication Management, plus education about Lifestyle Choices, Exercise Plans, making changes to your home environment, and charting day-to-day progress. Addresses other illnesses such as heart failure and high Blood Pressure.
Wound and skin care including
Gastrostomy and Colostomy Care:
Acute or chronic wounds can be the result of a health condition, an illness, or an injury. Caring for these types of wounds or a recurring skin issue can be complex. You may need specialized help, especially if an underlying health condition compromises care or contributes to prolonged healing.
